Patient Navigation
Ulman’s Patient Navigators work with the multidisciplinary medical care teams of our collaborating cancer centers, and with partners across the cancer community, to address the unique needs of young adults living with and surviving cancer in a holistic way.
NO MATTER WHERE YOU ARE – WE’VE GOT YOUR BACK.
To meet patients where they are, we have embedded Adolescent and Young Adult (AYA) specific navigators throughout the DMV to better connect patients, providers, and local resources through Community Navigation. Send us an email at communitynavigation@ ulmanfoundation.org
